GP Referral

Get back to fitness with GP referral

The Active 4 Health programme provides support for those with medical conditions who are looking to become more active and manage their health.

Clients are referred by the doctor or health professional and will attend an initial consultation with our fullly qualified instructor.

The instructor will decide with you the best options to help you become active and improve.  Activities can vary from gym, aqua therapy or specialised condition specific classes.

You will receive an end assessment after 12 weeks and look at the options to keep you on your active pathway to continue that change 4 life.

Bournemouth hospital meet two times a week for COPD rehab and Poole heart club meet twice a week.
